Subject: Medical Record Destruction for the Year 2025


Vibhavadi Hospital will proceed with the destruction of outpatient and inpatient medical records for the year 2025 in accordance with the hospital’s internal data control policy. The destruction process is scheduled to take place in May 2025, involving approximately 41,657 records, under the following conditions:

  1. Outpatient medical records of patients whose last visit to the hospital was in the year 2014 and/or those who have had no contact with the hospital for a period of 10 years.
  2. Inpatient medical records, including records of newborns admitted in 2014, who have had no contact with the hospital for a period of 10 years.
  3. Deceased patients' records for those who passed away more than 10 years ago (before the year 2014).
  4. Medical records related to legal cases, such as assault or traffic accidents, exceeding 10 years (before the year 2014).

In cases where medical records are required for legal purposes or ongoing legal proceedings, patients or concerned parties may request the hospital to retain such records. Requests should be made to the Medical Records Department at 02-561-1111 ext. 2006, during business hours (Monday to Friday, excluding public holidays, from 08:30 AM to 04:00 PM). Requests must be submitted by April 31, 2025.

After this deadline, the hospital reserves the right to proceed with the destruction of medical records in accordance with standard procedures.
